The Registrar

Dr. Jocelyn C. del Mundo is the current University Registrar of the University of the Philippines Manila. Her term as a University Registrar started in November 1, 2014. She is a professor from the Department of Behavioral Sciences, College of Arts and Sciences, UP Manila.

FREQUENTLY ASKED QUESTIONS
How to apply for a copy of Transcript of Records, Certification of Graduation, English Translation of Diploma, Certified True Copy of Diploma, Certified Text of Diploma, Honorable Dismissal, Company Verification, etc.

For the copy of Transcript of Records, kindly fill out the form and give it back to the person in-charge at Window 2. For first time applicants, it will take 3-4 weeks with clear record/with clearance, 4-5 weeks without clearance while for a recopy, it will take 2 to 3 days to claim. Kindly follow it up via phone a day before you claim. You may download the application form at our.upm.ed.ph

For the certification of Graduation, English Translation of Diploma, Certified True Copy of diploma, Certified Text of Diploma, Honorable Dismissal, Company Verification, you may claim the said documents within 2 to 3 days. For Certified True Copy of Diploma, you have to submit the photocopy of your diploma or the original. In some cases, Company Verification may last for about a week or more since it depends the payment of the company.

How to apply for transfer if coming from other UP units or from other school?

APPLICATION POLICIES FOR SHIFTEES AND TRANSFEREES

Shiftees are (1) students from a campus or Constituent University (CU) (e.g. UP Manila) who wish to enroll in another college (e.g., Pharmacy to Public Health) of the same campus or; (2) students from within a college who wish to get into a degree program other than that which they were originally qualified for (e.g., B.A. Philippine Arts to B.A. Political Science of the College of Arts and Sciences).
Transferees are (1) students from another Constituent University (e.g. UP Los Baños) who wish to enroll in UP Manila or; (2) students from another university who want to enroll in UP Manila.

A. Requirements
1. Students from another university must have passed at least thirty three (33) academic units
while students from other Constituent University must have passed at least thirty (30) collegiate
academic units. A student who has already earned more than 90 units, or is affected by
the Maximum Residency Rule (MRR) of the University, shall no longer be eligible
for shifting or transferring.

2. The applicant who wants to transfer must meet not only the minimum General Weighted
Average (GWA) of the University (2.00) but shall also satisfy the other admission
requirements of the accepting degree program/College (GWA, interview, examination,
available slots, etc.)

2.1 College of Medicine and College of Allied Medical Professions do not accept
transferees nor shiftees.

2.2 College of Arts and Sciences:
2.2.1 Accepts shiftees from U.P. Manila and transferees from other UP campuses.
A GWA of 2.75 or better for all academic subjects/courses is required for all degree
courses, except for B.S. Biology where the GWA should be 2.00 or better.
2.2.2 For transferees, a GWA of 2.00 or better is required for all academic degree
programs except for B.S. Biology which does not accept transferees from other
universities.
2.2.3 For both shiftees and transferees, only incoming sophomores will be accepted to
the B.S. Biochemistry, B.S. Computer Science and B.S. Applied Physics programs.

2.3 College of Dentistry:
2.3.1 Accepts only shiftees from U.P. Manila and transferees from other UP campuses
during the first two years of Pre-Dentistry with a GWA of 2.5 or better.
2.3.2 Shiftees from U.P. Manila and transferees from other UP campuses may apply
for the third year level (1st year Dentistry Proper) provided that they have completed
the required Pre-Dentistry courses with a GWA of 2.0 or better.
2.3.3 Transferees from other colleges/universities may apply for the third year level
provided that they have a Certificate of Pre-Dentistry Completion issued by their
respective colleges/universities with a GWA of 1.75 or better.
2.3.4 Transferees who have obtained a B.S. degree in a health related course
(e.g. B.S. Biology) may apply for the third year level with a GWA of 2.00 or better.

2.4 College of Nursing
2.4.1 Accepts shiftees (UP Manila) and transferees from other UP constituent
universities only (UP Diliman, UP Baguio, UP Los Baños, UP Visayas and UP
Mindanao) with a GWA of 2.00 or better for the first year level only.
2.4.2 Qualified applicants will be ranked according to their GWA and will be interviewed by
the College Admission Committee.
2.4.3 The available slots per year are dependent on the corresponding number of students
who transfer and/or leave UPCN in the succeeding semester.

2.5 College of Pharmacy
2.5.1 Accepts transferees from other schools with a GWA of 1.75 or better and
shiftees/transferees from UP campuses with a GWA of 2.00 or better.
2.5.2 Accepts second degree applicants: non-UP graduate with a GWA of 2.25 or better
and UP graduate with a GWA of 2.50 or better.
2.5.3 Qualified applicants will be ranked according to the criteria (school of origin, GWA,
present course, grades in computational sciences and chemistry and reason for
transfer) set by the College and will be interviewed by the College Committee
on Admissions and Scholarships.

2.6 College of Public Health
2.6.1 For UP Shiftees; a GWA of 1.75 or better. Priority is given to applicants within UP System.
2.6.2 If there are available slots, then applicants from outside UP System will be
considered. A GWA of 1.5 or better, is given priority.
2.6.3 To be admissible to the second year level, a student should have finished at least 34
units, including the required courses.
2.6.4 For the third year level, a maximum of 66 units should have been completed including all the first and second year required subjects.

B. Procedure
1. Accomplish the Undergraduate Admission Application form (one copy per course
application only).

2. In a brown envelope, submit to the Office of the University Registrar the following documents together with the completed application form:
2.1 Official Transcript of Records (for 2nd degree applicants only) or True Copy of
Grades (for continuing students) properly authenticated/signed by the
Registrar/Dean and the grading system of the school attended. TOR or TCG must
be complete up to the last term of schooling.

2.2 Certificate of good moral character from either the Dean/College Secretary/Faculty
Member of the school currently enrolled in.
2.3 Official Receipt of Application Fee issued by the UP Manila Cashier
2.4 One (1) 2 x 2 picture

C. Return Service Agreement
The Colleges of Dentistry, Nursing, Pharmacy and Public Health are covered by the Return
Service Agreement (RSA).

The RSA is an absolute admission requirement for the accepted applicants to the above-
mentioned colleges. The agreement states that the student shall serve in the Philippines within five years after graduation.

How to enroll as a graduate student?

Inquire about the requirements at the National Graduate School for the Health Sciences. Please take the entrance examination as scheduled, then on the date of enrollment, please bring all the requirements and enroll on a specific dates.

How to enroll as a non-regular student?

Apply directly to the college offering the subjects you intend to take.

How to enroll using eSAIS registration?

If new undergraduate first year students, kindly go to the Office of the University Registrar on the scheduled date. The following has to be submitted:

1. Original and two (2) photocopies of Admission Notice
(copy mailed to you by OUR)

2. Original and two (2) photocopies of your fourth year
high school card (F138) duly signed by the head of your school. A rubber stamped facsimile should be countersigned by the principal or his/her authorized representative.
NO HIGH SCHOOL CARD, NO ENROLLMENT

3. Original copy of Transcript (Form 137) duly signed by the principal/head of school

4. Medical Certificate issued by the PGH Health Service.

5. Accomplished two (2) copies of student directory.

6. Original and two (2) photocopies of your Certificate of Live Birth
(NSO certified)

7. Signed Return Service Agreement
(except Incoming CAS New First Year students)

8. Two (2) recent pictures, colored, size 2”x2”.
Print and sign name at the back.

9. Mailing Stamps:
4-year course - eight (8) mailing stamps
5-year course & Intarmed - ten (10) mailing stamps
6-year course - twelve (12) mailing stamps

Place pictures and stamps in a small envelope. Do not seal. Cost of Stamps:
NCR (National Capital Region) = 8 stamps @P7.00 each; Luzon = 8 stamps @ P9.00 each; Visayas & Mindanao = 8 stamps @P12.00 each

Additional Requirements for Foreign Students and those with Dual
Citizenship :

a. Study Permit secured from the Office of Student Affairs, 3rd Floor, UP Manila
Student Center Bldg., Padre Faura Street.

b. Original and one (1) photocopy of Identification Certificate (Recognition of
Filipino Citizenship from the Bureau of Immigration).

If old students, kindly go to the college concerned, registration using eSAIS is done at your respective college.

How to cross-enroll?

From UP Manila to other UP units:

Get a cross-registration form, color blue, and have it signed by the department chair, college secretary and the University Registrar.

Get an available subject to the campus you want to cross-register and have them signed in your blue cross-registration form. Follow their enrollment procedures.

How to file a Leave of Absence (LOA)?

For those enrolled and not enrolled refer to the academic calendar, our.upm.edu.ph

What to do:

Get the Leave of absence (LOA) form from your College Secretary's office

Fill out the form completely. If you are not currently enrolled, write NOT ENROLLED
in the space provided for the subjects enrolled

If you are enrolled, indicate all your subjects, signed by your respective instructors with the class standing indicated

Be sure that the form has been signed by your College Secretary or the Dean and have your parents/guardian signed at the space provided giving their consent

Pay the fee of P150.00 at the UP Manila Cashier's office

Submit the accomplished form together with your official receipt on or before the deadline stated in the Academic Calendar

How to apply for re-admission after leave of absence (LOA)

Go to your respective college for the issuance of a College Re-Admission slip. Proceed for registration process.

How to apply for a UP Manila photo-ID
If student, kindly present the invoice plus the official receipt paid during the semester. Then, fill up the application form for ID and pay the amount of P130 at the cashier's office. Write your name on the log book provided for ID and have your photo taken. You can wait the issuance of your ID.

If employee, kindly present the accomplished application form duly signed by the Head of HRDO and pay at the cashier's office the ID fee of P130.00. Have your photo taken and wait for the issuance of your new ID.

REQUIREMENTS FOR FOREIGN UNDERGRADUATE STUDENTS
A. For New Freshman (newly graduate in high school)

REQUIREMENTS FOR ADMISSION:
Must take and pass the UP College Admission Test (UPCAT) exam. It is usually given every 2nd week, Saturday and Sunday of August of each year or
Maybe admitted through automatic admission in any program with available slots if he/she has taken any of the following international qualifying exams such as:

Scholastic Aptitude Test (SAT) – with a total minimum score of 1200 in Math and Critical Reading only

General Certificate of Education (GCE) – must have passed three (3) Ordinary Levels and two (2) Advance Levels

International Baccalaureate Diploma

Documents to be submitted for application
For UPCAT exam – Please email the Office of Admissions at their email address: oadms@up.edu.ph for information on their UPCAT schedule and application form or visit the website www.up.edu.ph

For Automatic Admission – submit the following:
Foreign application form (our.up.edu.ph)
Application fee of $20 (subject to change)
Official SAT result or GCE or IB Diploma
Official High School Transcript
Two passport size photos
If with enrollment gap: Affidavit of no Collegiate Enrollment prior to enrollment to UP Manila

If admitted as a new freshman through UPCAT or automatic admission, submit the following requirements:
1. Original and two (2) photocopies of
Admission Notice (copy mailed to you by OUR)

b. Original and one (1) photocopy of Identification Certificate (Recognition of
Filipino Citizenship from the Bureau of Immigration

B. For Foreign Transfer Students (Those coming from other Universities)
Requirements for Admission:
Must have a general weighted average (GWA) of at least 2.00 or higher
Must have earned at least 33 academic units or more
Must submit all grades in all subjects taken from previous schools/universities
Must not be currently enrolled upon application to UP Manila
Must meet the required GWA for the degree program he/she is applying for
Must submit his/her Transcript of Record(s) for evaluation purposes
Must fill out an application form
Must pay the application fee of $20 (subject to change)
Must submit two (2) passport size photos
If with enrollment gap, please submit an affidavit of no enrollment during the semester/year that you did not enroll

If you are admitted to a college, admission notice will be given to you then you can apply for a student visa at the Office of Student Affairs.

If admitted as a foreign transfer student, submit the following additional documents:
Official Transcript of Record (Original copy)
Student Directory (2 copies)
Two (2) passport size photos
Birth Certificate (Original and photo copy)
Medical Certificate from the UP Health Service
Study Permit from Office of Student Affairs
TOEFL result – with a score of 500 (paper based), 173 (computer based) or 61 (IBT) – only for those students whose medium of instruction in their previous school is not “ENGLISH”
